Just something else to be aware of. I don't understand why "biopsy-verified CD" has to be the gold standard, but I suppose that lends to the legitimacy and ability to replicate a study to verify the results.

Conclusion This study indicates that individuals with biopsy-verified CD suffer increased risk of subsequent ESRD.

You're quite correct. There was nothing in that study to rule out any increased risk of ESRD for any other group of cohorts, (such as non-celiac gluten-sensitivity, N-C G-S). All they did was to verify that an increased risk of ESRD exists for cohorts with "biopsy-verified CD", compared with controls. They definitely did not prove that there was no increased risk of ESRD for anyone with N-C G-S - they didn't even consider that possibility.
