I used the very simplistic letter that Gabes references in above thread. THANKS GABES! I figured simple is better and honestly the hospital letterhead takes up half of the page. My Gabes letter can be used for stadiums, airlines, and other things. I've also used the letter for business conference registrations etc. i.e. I find it helpful not specifically tied to air travel. Gabes letter is below.
Suggested wording for Doctors letter:
This was mine – please change to suit your intolerances
This patient has a medical condition which involves serious food intolerances; specifically she is unable to digest wheat/gluten, yeast, soy, lactose, raw fruit and vegetables and raw fruit.
I would be most grateful if you would respect these significant medical issues.

I've never actually presented my letter to TSA. I've had it on me early on but then I started traveling with no liquids or gels. i.e. I think if I was going to be traveling with a liquid or a gel I would go to airport 2-3 months in advance with your letter and talk to TSA in person to ensure your documentation will work.
I called the nurse at my gastro doctors office and said I needed a letter and faxed over the Gabes letter that they modified with my info. i.e. did not require a Doctor's visit. If you have to modify your letter you can probably call the GI office and get it modified without a site visit cost.
I think there are several folks who have traveled from the USA with the letter and gel pack. Hopefully they will chime in.
