Hi Guys.
I've never been able to figure this one out, maybe you can? I'm noticing the past few days (and recall that I've noticed it in years past) that when I'm not in remission, having an empty stomach seems to bring on gas pains and urgency. I'm eating a low carb, high fat diet so I only eat twice a day (which I love) but now that it's about time to eat dinner, there is gas pain, rumbling, and the need to stay close to a bathroom. I was able to successfully eat lunch 7 hours ago and there was no gas or BM, but now that my stomach is empty, I'm not feeling so great. I should eat but since things are brewing in my gut that's the last thing I want to do.
Any ideas why this happens? Is this because my intestines have nothing to work on right now so they are irritating my lining, or is this maybe a symptom of BAM? Seems strange to me.... normally when I'm flaring it's right AFTER I eat that I have urgency issues, but lunch stayed with me. 7 hours later and it's ready to leave me. :(

Gas and urgency when stomach is empty?

Diagnosed November 2004, Used Asacol and Lialda, sometimes worked, sometimes made it worse. Entocort always works but hate it. Remission only lasts 3-6 months and then back on Entocort. Enterolab test July 2017, now gluten free. Time will tell!

Those may be symptoms of poor digestion. MC patients lose some of their ability to produce normal amounts of digestive enzymes (even after they recover, in most cases). IOW, we may no longer be able to digest large meals, especially meals that contain a lot of complex carbohydrates. That means that we do better with more frequent, smaller, meals, and/or more protein/less carbs, so that we aren't as likely to run out of certain enzymes before we finish digesting a meal. When our digestive system runs out of enzymes, the rest of the undigested or partially-digested food spoils, so our body has to purge it.
If you're already on a low-carb diet, you may be low on stomach acid.
At least that's how I see the situation.

It is suspected that some of the hardest material known to science can be found in the skulls of GI specialists who insist that diet has nothing to do with the treatment of microscopic colitis.
Yes, been really low carb for 8 weeks now and am only eating 2, sometimes three times a day. I thought fasting and eating less often helped healing and gave the guts a rest.

I'm not so sure it helps healing (although research claims that it is a "healthier" way to live). IMO it mostly is used by some individuals as a way to reduce the frequency of trips to the bathroom. The main point is, if you are going to do that, you have to be able to digest larger meals, and I doubt that some of us can do that, especially when they are in a flare (due to the limited enzyme production ability).

You never cease to amaze me with all the knowledge and info you carry around in that head of yours! Actually the meals I am eating aren't very large.... the beauty of being in ketosis is that you aren't hungry often and the fat content of the meals fills you pretty quickly.
