Is Uceris Better Than Enterocort?

Post by Sorphal79 »

I have been taking Enterocort at full strength (9 mg per day) for four months now. There hasn't been much improvement (maybe only slightly). My GI doctor is considering putting me on Uceris instead. I know this is also Budesonide, but he says it works by releasing into the entire colon rather than a beginning release in the ileum and only part of the colon (which is what Enterocort does?).

Perhaps this may be worth considering since my biopsies results showed Lymphocytosis in both my right and left colon and my small intestine (from recent EGD) is completely normal. However, I am concerned that I have been on the Budesonide for too long and this is basically just a continuation. The other option he mentioned was to try Prednisone, although both he and I do not want to go that route if it can be avoided.

He was a little puzzled as to why the Enterocort hasn't worked for me since he said his other microscopic colitis patients saw improvement in about two weeks. Though, he did say the lymphocytic colitis usually harder to treat than collagenous.

Has anyone had any real success with Uceris? Is it worth trying if Enterocort has failed?

Also, is Prednisone effective in treatment MC?

Gastritis is not uncommon with MC, because any and all organs in the digestive system tend to become inflamed. While I suppose it might be possible that a corticposteroid might inflame the stomach, in most cases the inflammation is caused either by NSAIDS or by the disease itself, not by a corticosteroid

Your doctor is wrong about Uceris. Unless the enteric coating (controlling the release of the active ingredient) has been changed since it's release (which is unlikely because that would require retesting, re-approval, and relabeling), Uceris is released in the lower third of the colon (definitely not the entire colon), because it's labeled for use in treating UC (not MC). It usually works well for MC, however, and theoretically should be safer for the adrenals. Plain old budesonide is released in the terminal ileum (which is only a few inches in length, and usually inflamed in MC patients) and the entire colon, so he's got that bass-ackwards. Uceris is not likely to make any difference in the development of gastritis, however, because gastritus is inflammation of the stomach. The stomach is roughly 20 feet ahead of the colon (almost 20 feet ahead of where conventional budesonide is released in the uleum).

Your doctor is also wrong about LC being harder to treat than CC. If there's any difference, it's CC that's harder to treat, because it's a more mature form of the disease. Sounds as though your doctor could stand a little brushing up on treating MC, and the medications used to treat it.

It is suspected that some of the hardest material known to science can be found in the skulls of GI specialists who insist that diet has nothing to do with the treatment of microscopic colitis.
