Getting off PPI - can cause a flare??

Post by nclarkramirez »

Hi all,
So I'm finally trying to get off PPIs after being on them for many years. I'm weaning slowly and doing a functional medicine protocol where you take the PPI one day and H2 blockers the next. I am only one week into doing this every other day thing and last night I had one of the worst flares in 2 months...Went about 7 times in 10 hours with 4 or 5x being being a good amount of WD and a few times being gas with bits of WD and stool. The D did feel acidic. Food has not changed--still very restricted. It was an H2 blocker day yesterday and I didn't eat as much. I'm wondering if it really was the acid that caused flare. So today I'm trying to exist on little sleep which I know doesn't help either. I had to finally take immodium this morning and it actually worked this time and haven't had WD since. I'm worried about tonight. Has any one had any experiences like this? Coincidentally, I had the one crown I have (no other teeth probs) fall off yesterday and had it fixed this morning. I thought that might have been the reason for flare, but was told there was nothing wrong with crown, no decay etc. (It likely fell off due to invisalign)
I can see why people fail getting off PPIs. I can feel acid as I write this and today was a PPI day.

PPIs cause so many MC problems, that in my opinion, prescribing them should be illegal, or at least reserved only for emergencies uses. I assume you're aware that H2 blockers are only effective for a couple of hours, so you have to take them relatively frequently, rather than only one per day. And frankly, PPIs are so prone to triggering MC, but I'm not sure I would have much confidence in a program that allows them to be used every other day, because their effect lingers for several days. As you've discovered, weaning off them can be really tough.

Some dental procedures can trigger a flare, especially if they involve the use of cleaning compounds (because some cleaning compounds contain gluten), or the use of an antibiotic. But in addition to this, dental procedures tend to be highly stressful for many individuals, and stress alone, is enough to trigger any MC flare.

I hope this helps.

Dental procedures cause reactions for me

the combo of the anaesthetic, the stress etc.
Also some of the rinse solutions etc can have alcohol and other triggering ingredients in them,

regarding migrating off PPI's. Magnesium and B6 are the key nutrients the body needs to 'balance' stomach acid
have you been supplementing with these supplements?

My elderly mother had been on PPI's constantly for over 10 years, we transitioned her off them. for a few months we increased Vit D3 and magnesium intake, and then as we reduced the PPI dosage increased magnesium and B6 intake
it took 2 months to do the transition
she only has very mild heartburn when she eats certain ingredients when eating out

The increased acid can contribute to flare - one option that may work for you is colestyramine which reduces bile acid during digestion
(none of the harmful impacts of PPI's). colestyrramine is one of the recommended medications for MC
there are quite a few discussions about BAM (bile acid malabsorption) and its link to MC that you can find using the search function

Thank you, Gabes & Tex!

I'm getting off the PPIs no matter what but I know for me, I need to do it so it's manageable since it's been like 8+ years. Thankfully, I've always been on the lowest dose of raberzole and I have a lot of esophageal soothing supplements that can help...still not super fun but I'm determined. I'm taking it one day at a time.

Daily, I take Doctors Best Chelated Mag 200mg, 5000 Vit D3, and a B-complex than includes 16.7 mg of B6. Melatonin has also been shown to help with GERD over time.

I was actually prescribed Colestipol but I have yet to actually try it. I'm not sure why, but I have been very afraid to try it!!

I was doing pretty well for awhile...and having only 1-2 mostly solid bm in the morning, but in the past few weeks, I am back to WD but just 2x every morning. The 7x in 10 hours was not usual and hasn't happened since. I think it may have been the stress like you and Tex said. I honestly didn't think I was stressed about it--but apparently I was....

Thanks again for the advice!

If you're not already using one, a sleep wedge pillow would probably help for resolving GERD issues. You can even get them long enough to go down to your waist, to eliminate any back problems.

Thanks, Tex -- That definitely does help--highly recommended. I ended up getting a bed that elevates and it very much helps
