Budesonide Question

Hi,

I have been a member to this board for a year or two. Great board with great advice, Most times I just read the board and find my answers. I have had MC for two years now. I took your advise on Budesonide and had a major flare up in Feb 2023. Started Budesonide again, with the normal dosage of 3 pill per day 3mg each. I slowly weaned off of Budesonide in Nov 2023. It was a very slow taper the last month 1 pill every 4 days, everything was good. In December 2023, I just had another flare up of 5 days of WD, I ate some dairy products, wasn't a lot but it caused a flare up.
The question I have is I just started Budesonide again only 2 pills a day. It stopped the WD within 2 days. I want to change to 1 pill a day and slowly wean off within a month. Do I need to go through the whole process of a 6 to 12 month wean off of budesonide. The flare up was from diary products which I was eating when I was on Budesonide, Now I know no diary products. Thanks for a great board, with great information.

Hi Roland,

We're all different, so it's difficult to accurately predict what will work specifically for each one of us, but casein antibodies only have a five or six day half-life, so it doesn't take very long to recover from the damage caused by a casein triggered reaction. Gluten, by comparison, triggers antigliadin antibodies, which have a 120 day half-life, and a reaction tends to cause a lot of intestinal damage. Healing all that damage takes much longer (than healing the damage caused by most other food sensitivities). So obviously a taper following the reintroduction of budesonide because of a gluten reaction has to be much, much longer than the taper following a casein reaction. So realistically, unless your casein antibody level is especially high, a taper of a few weeks should be sufficient, if your diet is squeaky clean. Listen to your body, and if it doesn't seem to feel quite safe, yet, you might want to extend the taper for another week or two, for extra security. If you feel the need for even more security, taking an antihistamine each day for a week or two after a budesonide treatment is ended will help to prevent a relapse.

As you have found, budesonide will sometimes mask the reactions due to certain foods, unless our immune system is extremely sensitive to those foods.
