Hmmmmmmmmm. I have several observations.
The way I see this, the hospital is probably correct that the use of the hydraylic fluid did not endanger any patients. However, the failure to use a good sterilizing soap on those tools probably DID endanger some of them, due to failure to remove prior existing contamination of the tools. Also, if the tools still felt slick, then clearly, their subsequent steam cleaning procedure is inadequate to remove soap residue, if it did not remove the oil residue.
Whoever used that hydraulic fluid as soap, and didn't notice the difference, (for several months, to boot), shouldn't be allowed near sharp objects, and shouldn't be allowed to feed themselves, nor to drive a motor vehicle, let alone work in a hospital. Used hydraulic oil is distinctly different from liquid soaps. It differs in odor, color, texture, specific gravity, viscosity, and virtually every other paramater by which fluids are distinguished from one another. Whoever used it had to totally disregard multiple red flags.
The elevator company is liable, because it is illegal to knowingly mislabel hazardous materials, and it also illegal to abandon them, or otherwise improperly dispose of them.
Everyone was at fault, except the poor patients. Sigh.
