It's our genes. Apparently, genes not only determine our susceptibility to certain diseases, but other genes determine the options that we have available to treat those diseases. Genes are the reason why we always have to remind ourselves that we are all different, and no treatment is effective for everyone. For example, this study shows that patients with the HLA-DR 8 gene do not respond to treatment with budesonide, (Entocort). (Note that this is not the same gene as the HLA-DQ8 gene that confers gluten sensitivity to some celiacs).
