An Association Between Microscopic Colitis and Celiac Disease.

BACKGROUND & AIMS:: Microscopic colitis (MC), characterized by non-bloody, watery diarrhea, has been associated with celiac disease (CD). We aimed to determine the extent and significance of the relationship between CD and MC. METHODS:: A prospectively maintained database of CD patients, seen between 1981 and 2006, was analyzed. Standardized morbidity ratios (SMR) were calculated using a general population study of MC as the reference group. Statistical analysis was conducted using the Student t-test, Pearson (2) test or Fisher exact test, where appropriate. RESULTS:: MC was found in 44 (4.3%) of 1009 patients; this represented a 45-fold increased risk for individuals with CD have MC, compared to the general population (SMR 45.5(CI 27.7-63.3). The CD patients with MC were older (p = 0.0001) and had more severe villous atrophy (p =0 .002) than the CD patients without MC. MC was diagnosed after CD in 64% of the patients, simultaneously in 25% and before CD in 11% (p=0.0001). Pancolitis predominated, though 16% had colitis limited to the right colon. Steroid or immunosuppressant therapies were required in 66% of the CD patients with MC and as maintenance therapy were given to 50% of these patients. Follow up biopsies revealed that the colitis persisted in 57% of the patients with CD and MC, despite improved diarrhea symptoms. CONCLUSIONS:: MC is more common in patients with CD than in the general population. Patients with CD and MC have severe villous atrophy and frequently require steroids or immunosuppressant therapies to control symptoms.
