hi :) I have dental surgery (bone graphing) on the 19th and my dentist is suggesting antibiotics 24 hrs prior with a 5 day post dose. Last time (during my extraction of a tooth) he did the same thing and it contributed to a flare (I think.) Currently I am taking 3 mg of budesonide every 2 or 3 days. Any suggestions to prevent an antibiotic related flare?

About 10 or 12 years ago I had a couple of root canals done, and because it took so long (several appointments), I had to have three back-to-back antibiotic treatments. Bactrim was prescribed, and I did okay on the first two regimens, but when I started the third, I had an allergic reaction, so I switched to ciprofloxacin. Cipro works well, but it has a lot of black box warnings because it severely depletes magnesium, and since most of us are magnesium deficient to begin with, because of MC, it's difficult to keep our magnesium level up to a safe level when taking Bactrim, because the magnesium has to be split up to several doses per day, so that we can absorb it, and a magnesium dose has to be taken either four hours before or at least two hours after taking Bactrim or it will interfere with the efficacy of the Bactrim. Anyway, that worked for me.
Since then, I've taken Cipro whenever having dental work done. But about a year or two ago, I had a reaction to it, also, so I had to stop taking it. I used azithromycin (Z pack). I'm not sure whether azithromycin will work for that procedure, but I suspect it should. Your oral surgeon might object to it, though, since it's surely not his first choice. When I used it, I was having a tooth pulled, which is rather similar, since it leaves the bone exposed during the procedure. It worked well for me, and in fact, I haven't had to see a dentist since then. But your oral surgeon will surely have the last say, since he or she can refuse to do the procedure if he or she doesn't trust the antibiotic. Sometimes we have to do what we have to do, unfortunately.
